Model of Care

Our Centre is a place of healing, education and wellness, offering treatment services uniquely tailored to meet the needs of each individual client. The service and treatment model fits with Canadore College’s vision for The Village that blends Indigenous, Eastern and Western practices, all embracing the concept of a healthy mind, body and spirit within a health and wellness continuum of care model that is inclusive and holistic.

The facility will be a 53-bed medical and therapeutic Centre with levels of care that include Residential Withdrawal Management Services, Residential Addiction Treatment and Residential Supportive Treatment for individuals aged 16 years and up. The Centre meets the unique needs of our vulnerable and diverse clientele including all genders, cultures and spiritual backgrounds.

The interdisciplinary team will design and provide client-centered care plans to meet the needs of our clientele within their recovery journey, beginning with their application for service and including post-treatment outreach monitoring to reduce or prevent potential relapses. Participants will receive medically monitored individual client-based programming within all service tiers, supporting and addressing primary care needs under the supervision of on-site physicians and a medical team.

Our therapeutic service models will incorporate a variety of Best Practices, evidenced-based recovery program modalities, including trauma-informed, CBT and DBT informed, person specific services / individualized care plans, with a bio-psycho-social-spiritual focus. The tiered model of services is to build therapeutic relationships with a humanistic and holistic approach, fostering a community of safety, connection, inclusivity, and cultural respect.

 

Residential Withdrawal Management Services

Our medical model will provide 24/7 support and entail a medical team of physicians and nurses who will provide medically monitored withdrawal services, addiction treatments, supports and primary care needs to individuals aged 16 years and up who are addressing complex and chronic substance use and related issues in an intensive rehabilitation setting. Services may be provided with or without the aid of medicated-assisted therapy, medication assessment and management, and / or other medical interventions including opiate replacement / opioid agonist therapy based on symptom management requirements. These treatments will be informed by Best Practices and individualized to patient medical needs and goals. All clients will be assigned a primary Addiction Therapist who will begin the treatment process during the client’s WMS stage and provide additional supportive services including daily counseling contacts with a bio-psycho-social-spiritual focus, early recovery education groups and transitional / discharge planning.

 

Residential Addiction Treatment Services

The Residential Addiction Treatment program provides an intensive, time-limited, trauma-informed treatment in a structured, substance-free setting for all genders aged 16 years and up who are seeking a supportive environment away from their usual living situation to address their complex substance use related issues. Continuum of care services are designed to aid with stabilization and the planning of reintegration into their home community with a combination of group work, psychoeducation, life skills retraining and psychotherapeutically dynamic individual counselling. The Addiction Therapy team provides an intensive, structured, evidence-based model of care that is trauma informed and includes CBT and DBT therapeutic approaches. Every client will work with their therapist to develop an individualized care plan that will incorporate their bio-psycho-social-spiritual and cultural needs. Scheduled programming includes ongoing assessments, individual and group counselling, psychoeducation, meditation, transitional planning for community reintegration, referrals and access to community-based services and supports. On-site physicians and medical team will provide assessments, medication management including opiate replacement / opioid agonist therapy, and supportive primary care to individuals.

 

Residential Supportive Treatment Services

The Residential Supportive Treatment program provides a safe, substance-free environment with a low to moderate intensity of services in a positive, empowering model of care for longer term treatment of problematic substance use for all genders aged 16 years and up. This program is available to residents of Ontario who have completed a residential treatment program and who are seeking a supportive therapeutic environment away from their usual living situation to continue addressing their substance use and related psychosocial issues. Supportive treatment will address individual care plans with a focus on preventing and reducing potential relapse. Individuals accessing this service will have the opportunity to seek employment, participate in continuing education, and establish strong, healthy community support connections. Continuum of care services are designed to aid with post-treatment stabilization and reintegration into their home community with a combination of peer mentoring, group work, psychoeducation, life skills retraining and psychotherapeutically dynamic, trauma-informed individual counselling. Participants will have the benefit of ongoing post-treatment contacts with the therapeutic team.
