How to submit your application form:
- Open the PDF Form
First, open the PDF Application form using a PDF reader or editor like Adobe Acrobat Reader, Foxit Reader, or Preview (on macOS). - Fill Out the Form
Click on each field and type in your information. For checkboxes or radio buttons, click on them to select or deselect. - Save Your Progress
It is a good idea to save your progress periodically. This ensures that you do not lose any information in case of unexpected events like a software crash. Use the "Save" or "Save As" option in your PDF reader/editor. - Review Your Application
Before submission, review all the entries you have made to ensure accuracy and completeness. - Submit the Form
Once you have filled out all the necessary fields and reviewed your entries, look for the "Submit" button at the end of the PDF. Click on it, and it may prompt you to choose a method for submission, such as email or a web service. If the PDF does not automatically attach to an email, please send an email to info@northernaddictiontreatmentcentre.ca with the completed PDF form attached.
